There's a famous one (well, famous in the motor racing world) of Brian Redman in a Cooper Grand Prix car in about 1968, with his front suspension just beginning to buckle and Redman himself looking at it.  The resulting crash nearly killed him and he had amnesia, and everyone assumed the crash was his fault, until the photo was published.
