In 1993 I think more people would have guessed that was a bad drawing of The Undertaker rather than a passable drawing of Blackjack Mulligan.

McDowell's nickname was Blackjack.  Here's a photo of the reverse side.   There would have been no confusion with The Undertaker.

I just finished reading the new Sheik biography (well worth a read) and this picture is in the book. No real explanation other than this is from one of the FMW tours that Sheik did with Sabu at the end of his career.

I could be wrong but I believe Apter still has that interview on audio tape and has released chunks of it over the past ten years.

It doesn't seem like that picture is specifically Blackjack Mulligan then, just a generic Wild West gunslinger.
