One that comes to mind is the one many years ago when the Lions and Bears went into overtime, and coach Marty Morningwheg (sp?) won the coin toss and selected the windy direction of the field. Bears got the ball and won the game without the Lions getting the ball. My memory of the details are a little foggy but some of you might recall this one.

Giants vs Philadelphia.  Pisarcik (sp?) fumbling handoff to Csonka when all he needed to do was kneel on it. I remember they said Csonka even questioned it in the huddle.  Herm Edwards scoops up th football and runs it in for the TD.

Mario Tremblay leaves Patrick Roy in the game. After giving up 9 goals  Roy throws up his hands and skates off the ice. Traded 3 days later in one the most lopsided trades in NHL history.

1995-6 football season. Manchester City are facing relegation. They need to better the results of Coventry City and Southampton.

With a few minutes to go, Man City are drawing 2-2, and both Coventry and Southampton are also drawing. Somehow manager Alan Ball gets the impression that Southampton were losing, and passed instructions to the team to play keep-ball rather than looking for a winner.

Outcome was all three matches ended in draws, which put Man City in the relegation zone...

You don't have to go far to find a coaching blunder. Game 6 of last year's WS, Kevin Cash removes Blake Snell  from the game in the 6th inning when Snell is pitching a 1-0 shutout & has 9 strikeouts. 

The Rays end up losing the game 3-1.

It was the days before everyone had a cellphone so I assume no one in the Manchester City camp had one or at least had no one at the other relevant matches keeping them up to date. Scoreboards at soccer stadiums in the UK were very rudimentary compared to what we had at the time in the United States and still aren't at the same level for a variety of reasons. I'm betting that Maine Road didn't have a scoreboard showing scores from around the league. Manchester City's players only became aware of the fact they needed to score a goal when Niall Quinn, a Man City player who'd previously been substituted and learned about what was actually happening while in the dressing room, came running out of the tunnel and then started shouting the information to his teammates on the field of play.
